Position Overview

We are seeking a dedicated Registered Nurse (RN) to join our team full-time as a Field Nurse supporting patients across Manhattan and the Bronx. Key Responsibilities

  • Perform in-home skilled nursing assessments and visits
  • Develop, manage, and update individualized care plans
  • Educate patients and caregivers on conditions, medications, and self-care practices
  • Document care accurately and in compliance with regulations
  • Collaborate with interdisciplinary healthcare teams to ensure quality outcomes
  • Supervise home health aides when necessary

Schedule

  • Monday through Friday
  • 8:00 AM – 5:00 PM
  • Field-based travel across Manhattan and the Bronx

Qualifications

  • Active New York State Registered Nurse (RN) license
  • 1–2 years of nursing experience in home care, long-term care, or a related setting preferred
  • Excellent clinical judgment and communication skills
  • Reliable transportation for travel within boroughs
  • Bilingual in Spanish or Mandarin is a plus
  • Professional liability insurance is required
